This usually boils down to:
1) Lack of explicit consent from your contacts to mail them
2) Old, stale mailing lists
3) Purchased or harvested lists
4) Offline lists
Having lots of spam complaints and unsubscribes will ruin your sending reputation and can get your account suspended!
